Best Care LLC will provide SCC services to individuals with intellectual and developmental disabilities (IDD) in the community. SCC services provide individuals with opportunities to build relationships and natural supports in their community, continue to interact with members of the community, promote safety, security, and independence, and work on personal goals. Best Care LLC will provide services through typical community settings for the SCC learning environment; and allow individuals to receive services on a one-time basis or recurring basis based on personal needs. Services will be provided on a one-to-one basis or in groups of two (2) to five (5) persons.
Best Care LLC will utilize SCC services to provide individuals with experience accessing their community competently, confidently, regularly, and safely; expose individuals to a variety of cultural, social, recreational, volunteer, and employment opportunities; and educate individuals about ways to access their community in an informed manner. SCC services will teach individuals how to plan, choose, and learn while accessing their community and provide opportunities to practice these behaviors.
